７ Individual information (no need proof, hand write ok)

　 B:Previous 2 companies, address, phone, position

         employment period, boss name, job description
　 C:Parents (Name of mother and father, date of birth)
　 D:Email address, Phone number

　 E:Rough travel date and city

     F:Countries that you have been to within 5 years 

        (Including Hongkong, Macau and Taiwan)
